Applications are available in any Alpena Regional Medical Center department including:  Registration/Front Lobby, Emergency, Social Workers, Surgery, the Cancer Center, Second and Third Floor stations.  

 

Medical personnel (patient's physician, nurse, social worker, ARMC managers/department heads, or other attending medical personnel) are required to sign and certify the patient's medical appointment.
